This procedure allows you to retrieve the forms you submitted on this website, as long as they are kept (retention policy applies).

If you have an account in this website, first login here, then enter your e-mail address below.

If you do not have an account you will be given a 6 digit PIN code:

  1. The PIN is valid for 2 days (or until you request a new one).
  2. An e-mail with links for every existing application form is send to your e-mail account.
  3. When opening these links, you will be asked to apply your PIN-code to gain access to your personal private data.
