• Form OF-156 (and Form DS-157 if you are male between the ages of 16 and 45)
• Valid Passport
• Two photographs
• Application fee
• Documents showing the purpose of your trip
• Documents showing your plan to remain in the U.S. for a limited time period
• Documents showing that you have a residence in your home country and other binding ties that will draw you back
• Documents showing that you will be able to support yourself financially while you are in the U.S.
